DRIVER’S LICENCE REQUIREMENT

A valid driver’s license and reliable transportation are required for ALL positions at James E. Fulton & Sons. Due to the nature of business at James E. Fulton & Sons, employment requires the ability to move from one jobsite to another as deemed necessary by supervisors.

EMPLOYMENT DRUG TESTING

James E. Fulton & Sons is committed to achieving and maintaining a drug-free work place and as a result:

ALL FINALIST FOR EMPLOYMENT MUST SUBMIT TO A PRE-EMPLOYMENT DRUG SCREEN

Any applicant receiving a positive test result will not be hired and may not reapply for employment with James E. Fulton & Sons, Inc. for a period of 1 year.

The only exception to this rule shall be for any applicant using a controlled substance prescribed by a doctor and the applicant has given the Human Resources Department prior notice. The prior notice must include an official written note from their doctor confirming the authorized prescription and that such substance does not impair safe and/or effective work performance.

The testing process will ensure individual privacy during the collection process and confidentiality of test results

I certify that answers given herein are true and complete to the best of my knowledge. I authorize James E. Fulton & Sons to investigate all statements contained in this application for employment as necessary to arrive at an employment decision.

I hereby understand and acknowledge that, unless defined by applicable law, any employment relationship with this organization is of an "at will" nature, which means the employee may resign at any time and the employer may discharge an employee at any time with or without cause. It is further understood that this "at will" employment relationship may not be changed by any
written document or by conduct unless such change is specifically acknowledged in writing by an authorized executive of this organization. In the event of employment, I understand that false or misleading information given in my application or interview may result in discharge.

I understand that I am required to take and pass a drug screening prior to an offer of employment and my employment is contingent upon the results of the screening. I understand that use, possession or sale of illegal substances in the work place will result in immediate termination and I hereby agree to be tested for the use of illegal substances upon request of this company. I also understand that if employed, I am required to abide by all rules, regulations and policies as set forth by the employer.

I understand and agree to the terms of this statement and any questions I may have had about this statement’s content and intent have been answered and I understand its terms.

Insurability under James E. Fulton & Sons auto coverage is a condition of employment for positions that require driving company vehicles. Driver selection is based on the criteria of our insurance carrier that demands a valid driver’s license and clean driving record.

Only authorized drivers are allowed to use company vehicles. Utilization by unauthorized drivers or non-employees is prohibited.

Company vehicles are to be used for James E. Fulton & Sons business only and are not to be driven for personal use. Driving James E. Fulton & Sons vehicles while under the influence of alcohol or illegal substances will result in immediate disciplinary action including termination.

A review of your driving record will be conducted annually and as appropriate, to determine your continued eligibility to be insured under James E. Fulton & Sons auto insurance policy.

Violation of James E. Fulton & Sons Driving Policy, repeated accidents, unsafe and irresponsible driving habits and a poor driving record will result in the loss of your driving eligibility with James E. Fulton & Sons and disciplinary action including termination.

I am aware that a motor vehicle report may be obtained as part of James E. Fulton & Sons evaluation of my job application
and/or employment requirement. The report may be procured by James E. Fulton & Sons or it’s insurance company
representative(s) and may include personal information obtained from state motor vehicle departments that may include my
driving record and assessment of my insurability for the insurance program or other consumer reports.

By signing this letter, I hereby provide my authorization for James E. Fulton & Sons, Inc. to procure such information and
reports, as well as additional reports about me from time-to-time as deemed appropriate, to evaluate my insurability or for other
permissible process.
